Surveys: During the onboarding process, users complete surveys related to health priorities (e.g., sleep, diet, exercise, mental health, focus).
Coach Sessions: Data is collected during virtual or face-to-face sessions with health coaches. These sessions may include transcriptions to enhance the user experience.
Wearable Device Integration: If users opt in, we collect data from wearable devices (e.g., steps, heart rate, sleep quality) through integrations with devices like Fitbit, Apple Watch, etc.
Please note, we do not actively collect sensitive personal data (e.g., racial/ethnic information, genetic data), unless you voluntarility provide it during sessions.
2. Data Use
The data we collect is used to:
Provide personalized coaching and recommendations based on your health goals.
Improve the user experience by tailoring the platform to individual needs.
Perform anonymized analysis for aggregate reporting (e.g., company-wide health trends). No identifiable data is shared without your explicit consent.
3. User Responsibilities
By using our services, you acknowledge and agree:
Coaching services are for general wellness purposes only and are not a substitute for medical advice, diagnosis, or treatment.
You are responsible for consulting a licensed healthcare professional before making health decisions based on coaching advice.
4. Limitation of Liability
To the fullest extent permitted by law:
Concorde Health and its health coaches shall not be liable for any direct, indirect, incidental, or consequential damages arising from your use of our services or reliance on coaching advice.
You agree to indemnify and hold Concorde Health and its staff harmless from claims or disputes resulting from actions taken based on our services.
5. Data Sharing & Retention
Your personal data is accessible only to:
Concorde Health Coaches: For the purpose of delivering personalized coaching.
Concorde Health Administrators: To maintain the platform and ensure smooth operation.
Employers will not have access to any of your personal or identifiable data unless you provide explicit written consent. Anonymized data may be shared in aggregate form for reporting purposes.
We retain your personal data indefinitely unless you request deletion. You may request data deletion at any time by:
Using the deauthorize and delete feature within the Concorde Health and/or Concorde Connect app.
Contacting us via email at admin@myconcordhealth.com.
Upon request, all data related to wearable integrations will also be deleted.
6. User Rights
Access: You may request access to your personal data at any time.
Deletion: You may request that we delete your personal data, and we will comply within 30 days of receiving your request.
Transfer: While we do not proactively offer data transfer to other providers, you may request it, and we will consider such requests.
If you revoke consent to this Privacy Policy, your personal data will be deleted within 30 days of your request.

By creating an account and using our services, you confirm that you:

Concorde Health does not knowingly collect data from children under the age of 18. If you are under 18, please do not submit any personal information to us.

All disputes related to our services will be resolved through binding arbitration in accordance with the laws of the Commonwealth of Massachusetts.
